Document Requirements
- Bali Visas application form.
- Child’s Indonesian & Foreign Passport.
- Child’s Identification Card (KIA).
- Scan of the identity and signature pages of the parent’s passport (no photos, must be a color scan).
- Child’s birth certificate or Akta Lahir.
- Indonesian ID Card (KTP) of parent(s).
- Family certificate (Kartu Keluarga).
- Scan of most recent visa sticker/stamp in passport of foreign parent.
- Marriage certificate of parents.
Renewal
- Renewal needs to be processed within 2 – 3 weeks before Affidavit expires.
- All previously required documents must be resubmitted as well as the Affidavit card itself.
